GRAYSON COUNTY, Va. (WRIC) -- Months before corrections officer Jeremy Hall was attacked by an inmate at River North Correctional Facility and died, a colleague reportedly warned Virginia Department of Corrections (VADOC) leaders of a "grave failure in oversight" with alleged endangerment of staff. On Nov. 17, corrections officer Jeremy Hall died after he was attacked by an inmate at River North Correctional Facility. Following the deadly attack, 8News sent two Freedom of Information Act (FOIA) requests regarding staffing to the VADOC. The first was denied due to security concerns, and the second was denied due to a lack of records. On Friday, Dec. 19, a joint federal lawsuit was filed in the Eastern District of Virginia Court against multiple VADOC leaders on behalf of Anthony Kellam, a current corrections officer at Indian Creek Correctional Center, and Dawn Hall, the widow of Jeremy Hall. The complaint in the lawsuit overviewed allegations of understaffing at River North on the day of the deadly attack, which 8News could not verify due to denied FOIA requests. In addition, the complaint alleged that VADOC staff knew of a "credible death threat" made by inmate John Holomon Russell before the attack, but alleged that no actions were taken. On the day of the attack, the complaint alleged that Jeremy Hall was assigned to work on the floor in proximity to Russell to supervise a trainee, but Hall was not a certified training officer, which is prohibited by the department’s policies. According to court documents, before the deadly attack on Jeremy Hall, Jacob Murray, a lieutenant at River North with over 12 years of experience at the department, sent an emailed resignation notice to several VADOC leaders that outlined multiple departmental concerns. 8News obtained the email, which was sent on July 10 -- roughly four months before the deadly attack -- in which Murray stated that "management's personal achievements and statistics on audits and EWP's supersede staff safety and morale." Murray further said in the email that he and colleagues believed that River North was "heading down a dangerous path," with the possibility of staff being "severely injured, if not killed." "I won't be a part of such a grave failure in oversight," the email said. 8News contacted Murray regarding the emailed resignation and has not heard back, as of reporting. Regarding Murray's resignation, VADOC said it could not comment on the active case in the following statement provided to 8News: "Thank you for contacting the Virginia Department of Corrections (VADOC). The VADOC cannot comment on active litigation." The full email sent by Murray is available below, with the redaction of personal information. Good afternoon all. After careful consideration I've have decided I will no longer be seeking further employment with River North Correctional Center effective immediately. This email is to serve as my final notice and to inform you I will perusing other options at this time. Although I am greatful for my time within the department and I owe a lot of thanks to a lot of good people. People who have challenged me guided me, and helped a young man grown not only in the department but in life, but Unfortunately the time has come where I do not feel that staff are the priority of RNCC nor are they within the DOC any longer. I can not and will not work somewhere that management's personal achievements and statistics on audits and EWP's supersede staff safety and morale. It is my personal belief along with several other staff and supervisors that RNCC is heading down a dangerous path with see staff severely injured if not killed, and I won't be a part of such a grave failure in oversight. This has been a hard decision to make after almost 12 years in the department but one that I fell is best for myself and my family at this time. I harbor no ill intent towards anyone within the DOC but do hope that the administration sees their failures within the facility and improves them so that RNCC doesn't continue losing staff and supervision, contributing to already unsafe conditions.
